Village Volunteers is a 501(c)(3) non-profit organization whose mission is to provide support, services, social and educational programs, and advocacy for older adults to enhance the lives of our members and the quality of life for all in our community.
The Care Manager position is key to assessing member eligibility and needs at intake, leading a care team of volunteers to assess members changing needs, listening and learning from volunteer experiences with members, and networking with the community while advocating for the welfare of older adults. The Care Manager reports directly to the Executive Director and is supervised by the Executive Director.
Employment contingent upon a clear background check and 3 professional references.
This position requires the Care Manager to work 20 hours a week scheduled over 4 days at a salary of $25 per hour.
Responsibilities
- Serve as advocate for volunteers’ safety and members’ well-being
- Conduct an initial intake needs assessment for prospective members
- Identify services that will meet the member’s specific needs.
- Identify at-risk members and ensure that the Care Team follows up and initiates the reassessment
- Track the member’s changing needs
- Conduct, in coordination with the Member Services Manager, regular Care Team meetings and assure follow up or reassessments are conducted
- Receive and assure reassessment actions are taken regarding feedback and concerns from volunteers and staff related to any changes with the member
- Communicates with members and caregivers as warranted by members’ change in condition
- Create a monthly activity summary and invoice, and submits to Executive Director at the end of each month for reporting to DSAAPD
Qualifications
- Bachelor degree, desired
- Nurse, social worker, or experience in Human Services
- Minimum two years of experience in care coordination, case management, social work, or aging services, desired
- Valid driver’s license, vehicle insurance and reliable vehicle
- Proficient in use of Microsoft Office applications, email and phone communications
Required Skills
- Proficient in verbal and written communication, and basic electronic forms of communication
- Strong interpersonal, organizational and problem-solving skills
- Ability to conduct assessment, planning, implementing and coordinating of services for members needs
- Ability to collect data, perform data analysis and management, and provide various methods of reporting
- Ability to work independently and also lead teams
- Ability to maintain confidentiality of members information and situations
- Ability to perform work in a professional, reliable and self-motivated manner
